PLEASE READ THIS FORM CAREFULLY BEFORE PROCEEDING. BY CHECKING THE BOX AT CHECKOUT, YOU ARE PROVIDING YOUR INFORMED CONSENT TO THE COLLECTION, TESTING, AND USE OF YOUR BIOLOGICAL SAMPLE AND ASSOCIATED HEALTH DATA.
1. Who This Form Applies To
This Authorization and Consent for Diagnostic Lab Testing ("Consent Form") applies to you if you are:
Ordering an integrative health lab test through Personomics, or
Enrolled in a Personomics coaching program that includes lab testing as part of your program
Lab tests facilitated through Personomics are fulfilled by EquiLife (equi.life), our sister company and lab partner. By consenting to lab testing through Personomics, you are also consenting to EquiLife's lab processing, handling, and results reporting procedures.
2. Voluntary Consent
I voluntarily consent and authorize Personomics, LLC and its lab partners — including EquiLife and their third-party laboratory providers — to perform the collection, testing, and analysis of my biological sample(s) (which may include blood, saliva, urine, hair, or other specimen types depending on the test ordered) for the purposes of functional and integrative health testing.
3. Risks, Benefits & Alternatives
Risks may include:
Mild discomfort, bruising, or irritation during or after sample collection
False positive or false negative test results
Emotional stress from unexpected results
Rare risk of sample degradation or processing error
Benefits may include:
Identifying potential root causes of health concerns
Providing data to support personalized health coaching and wellness planning
Tracking biomarker trends over time to support informed lifestyle decisions
Alternatives include:
Not obtaining lab testing at this time
Seeking testing through a licensed primary care provider or clinical laboratory
Using other testing methods or providers
4. My Acknowledgments & Agreements
By consenting to lab testing through Personomics, I acknowledge and agree to the following:
-I am the individual who will provide the biological sample for the test(s) I am requesting, or I am the parent or legal guardian of the individual for whom the test is being ordered
-I am at least 18 years of age (or the parent/legal guardian of a minor for whom I am consenting)
-I have read and understand the information provided about the test(s) on the website where I requested the test
-The information I have provided is accurate to the best of my knowledge. I will not hold Personomics, EquiLife, or their employees or agents responsible for errors or omissions resulting from inaccurate information I have provided
-I understand that lab results will be shared with Personomics, EquiLife, their third-party laboratory providers, and, where applicable, licensed clinicians involved in ordering or reviewing my results
-I understand that critical or abnormal results will be flagged in my results report. I assume responsibility for taking appropriate action regarding my results, including consulting my primary care provider or another qualified healthcare professional
-I will seek professional guidance if I have questions or concerns about my results, or if my health condition worsens
-I understand that lab tests expire within six (6) months of purchase date. I must collect and return my sample within 14 days of receiving my test kit
5. No Medical Advice or Physician-Patient Relationship
Lab services facilitated through Personomics and fulfilled by EquiLife are for informational and wellness education purposes only.
They do not:
-Constitute medical advice, diagnosis, or treatment of any medical condition, disease, or illness
-Create a physician-patient, therapist-patient, or any other licensed healthcare provider relationship
-Serve as a substitute for consultation with a licensed healthcare professional
You are solely responsible for your own healthcare decisions. Always consult a licensed healthcare provider before making changes to your medication, treatment, diet, or supplement regimen based on lab results.
If you believe you are experiencing a medical emergency, call 911 or your local emergency number immediately.
6. Geographic Restrictions
Lab testing through Personomics is subject to the following geographic restrictions, which are determined by our lab partner EquiLife and applicable law:
New York State: New York State law prohibits the shipment of lab products to New York State residents. Lab services are not available to you if you reside in New York State
Washington State: Lab services are currently suspended for Washington State residents. This suspension is temporary and subject to change
International: Lab services are available internationally only to residents of Australia, Canada, New Zealand, and the United Kingdom. Lab services are not available to residents of any other country
P.O. Boxes: We are unable to deliver lab kits to P.O. Boxes. A valid street address is required for all orders
By consenting to lab testing, you confirm that you do not reside in a restricted jurisdiction. If you are located in a restricted jurisdiction and proceed with a lab order, Personomics reserves the right to cancel your order and issue a refund.
7. Sample Collection & Return
You are responsible for following the collection instructions included with your lab kit
Domestic (U.S.): Use the prepaid domestic shipping label provided with your kit to return your sample. HTMA and Omega samples should be returned via regular mail with a postage stamp
International: You are responsible for arranging and paying for return shipping of your sample to the lab. Please contact your local post office for the most appropriate shipping method. Tracking is strongly recommended
Important: The name used to purchase your lab test must exactly match the name listed on your submitted sample. If the test is being completed by someone other than the purchaser, please notify us at [email protected] in advance so we can update our records. Failure to do so may result in a significant delay in receiving your results
Personomics and EquiLife are not responsible for samples lost, damaged, or degraded due to improper collection, packaging, or return shipping
8. Privacy & Your Health Information
Your lab results and associated health information constitute Protected Health Information (PHI) and are handled in accordance with our:

Your information may be disclosed to:
-EquiLife and their accredited third-party laboratory providers, for the purpose of processing and reporting your results
-Licensed clinicians or healthcare providers involved in ordering or reviewing your results
-Personomics health coaches and concierge team members involved in your program
-County, state, or other governmental and regulatory entities as required by applicable law
We do not sell your health information. Your identity is not for sale.
9. Authorization for Communications
By consenting to lab testing, I authorize Personomics and EquiLife to contact me via email and, if I have opted in, via text message (SMS) to communicate with me regarding my lab order status, sample receipt, and results. I understand that text messaging is not a fully secure means of communication and that my information could potentially be accessed by an unauthorized third party. I may opt out of SMS communications at any time by replying STOP.
10. Release of Liability
To the fullest extent permitted by law, I hereby release, discharge, and hold harmless Personomics, LLC and EquiLife, including their respective officers, directors, employees, representatives, agents, and third-party laboratory partners, from any and all claims, liability, and damages of whatever kind or nature arising out of or in connection with:
The collection, processing, analysis, or reporting of my diagnostic test
The disclosure of my test results as described in this Consent Form and our Privacy Policy
My reliance on or use of my test results without consultation with a licensed healthcare provider
Delays, errors, or omissions resulting from inaccurate information I provided
11. Refunds & Cancellations
Lab test orders may be cancelled and fully refunded if cancelled before your kit has shipped.
Once a kit has shipped, refunds are not available unless the kit is defective or lost in transit.
Personomics reserves the right to cancel any order placed by a resident of a restricted jurisdiction (see Section 6) and will issue a full refund in such cases.

12. Acknowledgment of Consent
By checking the box at checkout, I confirm that:
-I have read and understand this Authorization & Consent for Diagnostic Lab Testing
-I voluntarily consent to proceed with the lab test(s) described in this form
-I confirm that I do not reside in New York State, Washington State (while suspended), or any other restricted jurisdiction
-I am at least 18 years of age, or I am a parent or legal guardian consenting on behalf of a minor
-I understand that this consent will be recorded with my account at the time of purchase
By checking the box at checkout, you provide your informed consent to all terms described in this Authorization & Consent for Diagnostic Lab Testing. This consent will be recorded with your account at the time of enrollment or purchase.
